Membership fees are set by a consistent formula that considers age, the complexity of care, and whether you join as an individual, a couple or a family. Pricing is discussed during the consultation, with monthly or annual payment options available.

Membership fees are set by a consistent formula that considers age, the complexity of care, and whether you join as an individual, a couple or a family. Pricing is discussed during the consultation, with monthly or annual payment options available. The fee covers your relationship with Dr. Sisodia: extended visits, same-day or next-day appointments, direct phone and text access, and coordination of your wider care. It is not insurance and does not replace it.
It is buying a smaller denominator. The practice is capped at 150 patients. A traditional primary care panel runs into the thousands, and larger concierge practices publish panels in the hundreds. Every promise below follows from that cap rather than from goodwill.

How your fee is worked out
This allows the fee to reflect each patient's anticipated level of care. A healthy patient with minimal needs may pay less than someone requiring frequent visits and coordination with multiple specialists.
Which means the honest answer to what it will cost you is that it depends, and the practice would rather say so than quote a figure most members do not pay. You will have it in writing at the meet and greet, before anything is signed. Call (727) 607-0504 if you would rather ask first.

Does insurance or Medicare cover the membership fee?
Insurance and Medicare do not cover the membership fee. The fee is paid directly to My Choice Primary Care and is not billed to insurance, and Dr. Sisodia has formally opted out of Medicare.
Do I still need health insurance?
Yes, you still need insurance. We do not participate in any insurance networks, including HMOs, Tricare Prime, or Medicaid. Your membership covers your care with Dr. Sisodia, and it does not pay for care delivered anywhere else. You will need insurance for emergency care, routine bloodwork and laboratory tests, imaging, hospital stays, surgery, and specialist visits and procedures.

The fee covers your relationship with Dr. Sisodia: extended visits, direct phone and text access, and coordination of your wider care. It does not replace health insurance.
- How many patients does the practice take?
- The practice is capped at 150 patients. A traditional primary care panel commonly runs into the thousands, and larger concierge practices publish panels in the hundreds. The cap is the reason same-day or next-day access and unhurried visits are possible at all.
- What happens if the practice is full?
- The practice is capped at 150 patients. If the panel is full when you get in touch, you will be offered a place on the waiting list.
- Can I reach Dr. Sisodia after hours?
- Dr. Sisodia is generally available directly to members after hours. There is no routine outside covering physician or nurse. In an emergency, call 911 or go to the nearest emergency room.
- Does Dr. Sisodia make home visits?
- Yes, when medically appropriate. Home visits within the practice's service area, Clearwater, Safety Harbor, Dunedin, Palm Harbor and Largo, are included in membership.
- How long is the first appointment?
- Your first appointment as a member runs 60 to 90 minutes. It is separate from the meet and greet, which comes before you join.
- What is not included in membership?
- Billed separately and not part of membership: medications, laboratory testing performed by outside laboratories, imaging, specialist care, hospital care, vaccines, and other services provided by outside facilities or clinicians.
- Does Dr. Sisodia have hospital admitting privileges?
- No. Dr. Sisodia does not hold admitting privileges at any hospital, so if you are admitted, the hospital's physicians direct your care there. She may make a courtesy hospital visit when feasible, but she does not serve as the admitting or attending physician.
