Care

Geriatric care planning

Care planning for older adults and the families coordinating it, with a physician who is reachable directly.

Geriatric care planning covers the practical coordination that families usually end up managing alone: specialists, medications, hospital connections and what happens next. Dr. Sisodia is reachable directly, which matters most in exactly the moments it is hardest to reach a practice.

What this covers

The areas this includes.

  • Geriatric and aging care planning
  • Medication review across every prescriber involved
  • Specialist, imaging and hospital coordination
  • Remote monitoring devices, where clinically indicated
  • In-person visits at the Clearwater office, or secure telehealth

The mechanism

Why the practice size matters here.

Most of the load in caring for an older parent is administrative, and it falls on the family: chasing a specialist, reconciling what four prescribers have written, working out who to call when something changes at six in the evening. The practice takes that coordination on, and the cap is what makes the physician reachable directly at the moment it is hardest to reach a practice at all.

Before you ask

Questions about this kind of care.

Do you offer care for teens or older adults?
Yes. We care for individuals age 16 and older, whether you are a teen, young adult, parent, or senior.
Are visits in person or virtual?
Both. Most visits take place in person at our Clearwater office, but secure telehealth is always available when it is more convenient or clinically appropriate.
