My Choice Primary Care

Care

Hormone, metabolic and weight optimization

Hormone, metabolic and weight care managed inside your primary care relationship, not outsourced to a separate clinic.

Hormone, metabolic and weight care sits inside your primary care relationship rather than at a separate clinic that does not see the rest of your health. Dr. Sisodia manages it alongside everything else she knows about you, which is the point of having one physician.

Included

What membership covers here.

  • Managed by the physician who holds your full history
  • Reviewed against your other conditions and medications
  • Adjusted over time rather than set once
  • No separate clinic, no repeating your story
  • Physician evaluation and follow-up included in membership

What this covers

The areas this includes.

  • Hormone Replacement Therapy (HRT)
  • Menopause and perimenopause care
  • Testosterone Replacement Therapy (TRT), when clinically appropriate
  • Thyroid and adrenal health
  • GLP-1 medications, when appropriate
  • Metabolic optimization
  • Sustainable weight management
  • Family planning and reproductive support

The mechanism

Why the practice size matters here.

This is the area most often sold as a standalone program by a clinic that sees none of the rest of your health. Here it sits with the physician who already holds your history, so it is reviewed against your other conditions and medications rather than alongside them. Dosing is adjusted over time in ordinary appointments, which means no separate membership and no repeating your story.

Cost

What membership covers, and what it does not.

Yes, you still need insurance. We do not participate in any insurance networks, including HMOs, Tricare Prime, or Medicaid. Your membership covers your care with Dr. Sisodia, and it does not pay for care delivered anywhere else. You will need insurance for emergency care, routine bloodwork and laboratory tests, imaging, hospital stays, surgery, and specialist visits and procedures.

Insurance and Medicare do not cover the membership fee. The fee is paid directly to My Choice Primary Care and is not billed to insurance, and Dr. Sisodia has formally opted out of Medicare.

Billed separately and not part of membership: medications, laboratory testing performed by outside laboratories, imaging, specialist care, hospital care, vaccines, and other services provided by outside facilities or clinicians.

The membership fee itself is not a published figure. Membership fees are set by a consistent formula that considers age, the complexity of care, and whether you join as an individual, a couple or a family. Pricing is discussed during the consultation, with monthly or annual payment options available. How membership is priced.

Before you ask

Questions about this kind of care.

Can you help with hormonal health and weight management?
Yes. We offer expert support for Hormone Replacement Therapy (HRT), Testosterone Replacement Therapy (TRT), metabolic optimization and sustainable weight management, all tailored to your unique physiology and long-term goals.
Do you treat menopause, perimenopause and low testosterone?
Yes. Dr. Sisodia diagnoses and treats menopause and perimenopause symptoms and prescribes hormone replacement therapy when appropriate. She diagnoses and treats low testosterone in men and prescribes testosterone replacement when clinically appropriate. She also actively manages thyroid disease.
Do you prescribe GLP-1 medications?
Yes, when appropriate. Weight management is part of your individualized primary care rather than a separate packaged program.
Is weight-management care included in membership?
Physician evaluation and follow-up are included in membership. Medications, outside laboratory testing, imaging and services from outside clinicians are separate.
What does the first hormone, metabolic or weight evaluation include?
Your medical and medication history, symptoms, weight and metabolic history, nutrition and physical activity, relevant laboratory testing, cardiovascular risk, sleep, and individualized treatment goals.
What ongoing monitoring is involved?
Follow-up on symptoms, vital signs, weight and body composition when relevant, medication response and side effects, and laboratory monitoring appropriate to your treatment.
When would you refer me to a specialist?
When the diagnosis is uncertain, the disease is unusually complex, treatment falls outside primary-care scope, specialized procedures are needed, or you are not responding as expected. That can mean endocrinology, gynecology, obesity medicine or another specialist.
